Real-time games are meant to be played a single sitting, whereas multi-day games are meant to be played out over the course of at least a few days.
Game creators define whether their games are real-time or multi-day.
They will also set the boot settings accordingly. Real-time games typically have boot times of several minutes, whereas multi-day games have boot times of over 24 hours.
