Are you wondering who decides which template for a game? That would be random, computer generated.
Or are you wondering who decided on the templates that can be chosen? That would be Fizzer.

the selection of the map is random from a list that fizzer created.
the list was originally created based on templates used in the seasonal ladder, and then there was a poll for replacing certain templates with new ones.
a new poll should happen at some point, atleast that's what fizzer promised at the time.
