First of all, there are not all real countries, but more regions. Sometimes it stays with a country, sometimes more (GB and Ireland, Netherlands and Flanders) sometimes less than a country (USA west, USA Central, USA East) but the answer your question:

Team captain post line up on forum, link match after created games, decide who play and which match, and usually is the one that when in teamgames was undecided whether to do one thing or another has the burden to decide. It is usually the strongest player of that team to be captain.
When Dunga make a chat for imform other team, he invite only captain that after will speak with member of that team.

As far as the 5v5(/7v7) is concerned, I would like to opt for the same template as the previous Nations Cup, namely the European Borders one. These games were easily the best part of the whole thing and required a LOT of skill and planning.

Furthermore, for this edition I opt for fewer games. Two 2v2 games and two 3v3 games was just too much and people got tired of playing it over and over again. I'd suggest two 1v1 games and one of 2v2/3v3/5v5. Make 3v3 and 5v5 worth two points, the rest worth one so it adds up to an odd total to avoid ties.

And yeah, no group stage as that takes far too long.
